Reporting Hazing
Thank you for reporting suspected hazing. No student should ever face belittlement, humiliation, or exposure to potentially harmful situations in the pursuit of inclusion in a group at UC San Diego.
If you are a new member of an organization, stay connected with friends outside of the group. If you suspect an activity may be hazing, talk to friends and family members about what you are experiencing and seek their advice. Refuse to participate in an activity if you suspect it's hazing, or quit the organization. Report any suspected hazing to campus officials, if you prefer.
Any member of the campus, whether affiliated with the offending organization or not, has a responsibility to report hazing. Click below to find the various methods of reporting possible incidents of hazing. The university takes allegations of hazing seriously. Reports can be anonymous in nature through our online reporting module.
Submit a Report of Hazing
After you click the link, select Non-Academic under Violation Type, and fill in the remainder of the form. Your report will be sent to the Center for Student Accountability, Growth, and Education for review. Please click Submit only once, and allow your browser time, or you may accidentally submit duplicate reports.
How to report suspected hazing incidents
Any person may report alleged Hazing, including anonymously, as follows:
- Hazing conduct by Student(s) or Student Group(s) may be reported to the Center for Student Accountability, Growth and Education (SAGE) by:
- Completing an incident report online at sage.ucsd.edu, OR
- Calling (858) 534-6225
- Filing a complaint online at UC San Diego Abusive Conduct in the Workplace Submission.

- Hazing crimes committed by any person may be reported to the UC San Diego Police Department by:
- Calling 9-1-1 (in case of an emergency) or (858) 534-HELP, OR
- Filing a Clery Campus Security Authority Report Form available at Clery.ucsd.edu.
- Hazing conduct by Academic Appointees, Staff, and Volunteers (including student employees acting in the course of their employment) may be reported by:
- Filing a complaint online at UC San Diego Abusive Conduct in the Workplace Submission.
- Hazing conduct involving any individual may also be reported by:
- Reporting the activities to the Locally Designated Official at LDO@ucsd.edu, OR
- Submitting a complaint in writing via the UC Whistleblower Hotline, OR
- Calling 800-403-4744.
- Certain employees have specific reporting responsibilities for Hazing conduct that may also violate other policies, including but not limited to the University of California Sexual Violence and Sexual Harassment Policy, University of California Anti-Discrimination Policy, University of California Abusive Conduct in the Workplace Policy, University of California Clery Act – Campus Safety and Security Policy, UC San Diego Reporting Child Abuse and Neglect Procedures, UC San Diego Workplace Violence Prevention Plan, UCSDHP 305.4 - Abuse Screening, Assessment and Reporting: https://pulse.ucsd.edu/policies/UCSDHPs/MCPolicy/305-4.pdf and UCSDHP 539 - Workplace Violence Prevention Program: Individuals should refer to those policies and procedures to understand their reporting obligations.
